Parkinson's Disease Physiotherapy: The Evidence for LSVT BIG
LSVT BIG (Lee Silverman Voice Treatment — Big) is the most rigorously studied physiotherapy intervention for Parkinson's disease. It involves intensive, high-amplitude movements delivered in a standardised protocol: 16 sessions over 4 weeks.
LSVT BIG trains patients to produce large, exaggerated movements — counteracting the hypokinesia (reduced movement amplitude) characteristic of PD. A 2012 RCT in Neurorehabilitation and Neural Repair found LSVT BIG produced superior improvements in gait, balance, and upper limb function compared to standard exercise.
Parkinson's disease physiotherapy requires a very specific approach. The classic Parkinson's problem is not weakness — it is reduced amplitude and speed of movement, and difficulty initiating movement (freezing).
