Knee OA in Elderly Adults: India's Biggest Orthopaedic Problem
Knee osteoarthritis affects over 45% of Indian adults over 65, making it the most prevalent musculoskeletal condition in the elderly population. It is the leading cause of pain and functional disability, affecting the ability to walk, climb stairs, sit and stand from low chairs, and perform daily tasks. Despite its prevalence, knee OA is frequently undertreated -- many patients either suffer without adequate treatment or are pushed toward knee replacement surgery before exhausting non-surgical options.
What the Evidence Actually Shows
Multiple high-quality trials show that supervised exercise therapy for knee OA produces pain reduction and functional improvement equivalent to knee replacement at 1-2 year follow-up, in patients with moderate OA. The GLAD programme (Good Life with osteoArthritis in Denmark) -- a structured 8-week exercise and education programme -- has been shown to be cost-effective and significantly reduce knee replacement rates.
Physiotherapy Programme for Elderly Knee OA
Quadriceps Strengthening
Quadriceps weakness is the single most modifiable factor in knee OA. Progressive strengthening using chair stands, leg press, and step exercises significantly reduces pain and improves function. Even patients with severe OA respond to progressive strengthening.
Aerobic Exercise
Walking (flat surfaces, appropriate footwear), cycling (stationary or outdoor), and aquatic exercise all reduce OA pain and improve cardiovascular health. We design individually appropriate aerobic programmes.
Pain Management
TENS, ice/heat, and manual therapy provide pain relief that enables more effective participation in exercise therapy.
