Waking Up With Neck Pain
Waking up with neck pain -- sometimes severe, with the neck locked in a tilted position (acute wry neck or torticollis) -- is a common presentation at Realign Rehab Clinic. It most commonly results from sustained awkward sleeping positions that stress cervical joints and muscles beyond their capacity, or from minor cervical joint dysfunction that was asymptomatic until a night of poor positioning tipped it into an acute locked state.
Why Sleeping Position Matters
Poor sleeping positions cause neck pain through: excessive lateral flexion (sleeping on one side with too-high or too-low a pillow), excessive rotation (sleeping face down -- the worst position for the cervical spine), and prolonged flexion (sleeping on multiple high pillows). All of these positions place sustained stress on cervical joints, discs, and muscles.
Acute Wry Neck (Torticollis)
Acute wry neck is a sudden, severe limitation of cervical range of motion, typically with the head locked in a tilted position. Usually caused by a minor cervical joint dysfunction (apophyseal joint locking) or acute facet joint irritation. Extremely painful and alarming but responds rapidly to physiotherapy -- often resolving significantly within one session.

Pillow Assessment and Advice
The correct pillow height fills the space between the head and the mattress when side-lying, maintaining neutral cervical alignment. For most adults, this means a medium-firm pillow of appropriate height for their shoulder width. Memory foam pillows can be helpful for maintaining consistent support.
