When to See a Physiotherapist First
For the vast majority of musculoskeletal conditions, a physiotherapist should be your first point of contact: back and neck pain, joint pain from arthritis or overuse, sports injuries (strains, sprains, tendinopathies), repetitive strain injuries, post-fracture rehabilitation (after medical clearance), neurological rehabilitation, and post-surgical physiotherapy.
When to See a Doctor or Orthopaedic Surgeon First
Suspected fracture (immediate): significant trauma with severe pain, deformity, or inability to weight-bear. Cauda equina syndrome: back pain with bowel/bladder dysfunction and saddle anaesthesia -- emergency. Rapidly progressive neurological deficit: increasing weakness or numbness. Signs of infection or tumour: night pain at rest, unexplained weight loss, fever with musculoskeletal symptoms. After failed physiotherapy: if 3-6 months of appropriate physiotherapy has not resolved the problem, surgical assessment may be warranted.
