Why Pregnancy Causes Back Pain
Lower back pain is one of the most common complaints during pregnancy, affecting 50–70% of pregnant women. As the uterus grows, the centre of gravity shifts forward, increasing lumbar lordosis and placing greater stress on the lumbar discs, facet joints, and surrounding muscles. The hormone relaxin, which loosens ligaments to allow the pelvis to expand for childbirth, also destabilises the lumbar spine and sacroiliac joints, contributing to pain.
Types of Pregnancy-Related Back Pain
Lumbar Pain: Central or bilateral lower back pain, similar to pre-pregnancy back pain but exacerbated by pregnancy posture changes. Pelvic Girdle Pain (PGP): Pain in the sacroiliac joints, pubic symphysis, or hip area. Often worse on one-legged activities (walking, stairs). A distinct condition from lumbar pain requiring different management. Sciatica During Pregnancy: Shooting pain into the leg. Can be due to disc herniation or piriformis syndrome aggravated by pregnancy posture.
Safe Physiotherapy During Pregnancy
Manual Therapy
Gentle lumbar and SI joint mobilisation (side-lying or seated to avoid lying on the back after 20 weeks) provides effective pain relief. Soft tissue techniques release paraspinal muscle tension.
Pelvic Girdle Belt
A maternity support belt or SI joint belt provides external stability to the loosened pelvic joints, significantly reducing PGP pain with movement.
Specific Exercises
Pregnancy-safe core exercises (clam shells, modified bridges, pelvic tilts) strengthen the stabilising muscles without excessive spinal loading. Water aerobics is particularly effective for pregnancy back pain.
Postural Advice
Sleeping with a pregnancy pillow (wedge under the bump, pillow between the knees), sitting posture correction, and lifting mechanics during pregnancy.
