Why Sitting Aggravates Back Pain
Sitting in a flexed posture (as most people do, regardless of ergonomic advice) increases lumbar disc pressure by 40% compared to standing, and by 90% compared to lying. This increased disc pressure is why back pain from disc-related conditions is almost always worse with prolonged sitting. Additionally, sustained sitting weakens core muscles and shortens hip flexors, compounding the problem over time.
Conditions That Cause Sitting Back Pain
Lumbar disc herniation: disc-related pain that worsens with sustained flexion and is typically relieved by standing or walking. Central lumbar stenosis: worse sitting, relieved by bending forward (paradoxically, walking can worsen stenosis but sitting in a flexed posture often relieves it). Lumbar facet joint pain: typically better with sitting, worse with standing and extension -- the opposite pattern to disc pain. Sacroiliac joint dysfunction: often worse with prolonged sitting, particularly asymmetric sitting.
