Causes of Morning Back Pain
Disc-Related Morning Pain
The lumbar discs rehydrate overnight (filling with fluid during non-weight-bearing), making them thicker and more turgid in the morning. This can temporarily increase nerve root pressure in disc herniation patients, causing worse morning symptoms that ease as the fluid redistributes through daily activity. Typically eases within 15-30 minutes of getting up.
Facet Joint Osteoarthritis
OA stiffness is worse in the morning (synovial joint stiffness from inactivity) and eases with movement -- the classic "gelling" phenomenon. Typically eases within 30-60 minutes of movement.
Inflammatory Arthritis (Ankylosing Spondylitis)
Morning stiffness lasting more than 1 hour, particularly in young men, should raise suspicion for ankylosing spondylitis. Unlike mechanical causes, inflammatory stiffness is worse at rest and eases with activity. Requires rheumatological assessment.
Sleeping Position and Mattress
