Ankle Sprains: The Most Recurrent Sports Injury
Lateral ankle sprains are the most common sports injury, accounting for 10-15% of all sports injuries. More concerning is their recurrence rate -- 40-70% of people who sprain an ankle will sprain it again, often multiple times. Chronic ankle instability (persistent giving way and recurrent sprains) develops in 30-40% of acute ankle sprain patients who receive inadequate rehabilitation. This high recurrence is almost entirely preventable with appropriate physiotherapy.
Why Ankle Sprains Recur
An ankle sprain damages not just the lateral ligaments (ATFL, CFL, PTFL) but also the proprioceptive nerve endings within these ligaments. Proprioception is the joint's position sense -- the nervous system's ability to detect ankle position and trigger protective reflexes before a full sprain occurs. Without specific proprioception rehabilitation, this deficit persists even after the ligament pain has resolved, leaving the ankle vulnerable to re-injury.
Physiotherapy Ankle Sprain Prevention Programme
Proprioception Training
Single-leg balance training on progressively unstable surfaces -- flat ground, foam pad, BOSU ball, unstable balance board. This retrains the neuromuscular reflexes that protect against ankle sprains.
Peroneal Muscle Strengthening
The peroneal muscles (on the outer side of the lower leg) evert the foot and are the first line of defense against lateral ankle sprains. Specific peroneal strengthening with elastic resistance significantly reduces re-sprain rates.
Functional Plyometric Training
Jumping, hopping, and cutting drills that train the ankle in sport-specific loading patterns before returning to sport.
