Headache Classification: Why Type Matters
The International Headache Society classifies over 150 different types of headache. For physiotherapy purposes, the most relevant distinction is between primary headaches (where the headache itself is the condition) and secondary headaches (where the headache is a symptom of another condition). Most headaches seen in physiotherapy practice are tension-type headache or cervicogenic headache -- both of which respond very well to physiotherapy.
Headache Types and Physiotherapy Response
Tension-Type Headache (TTH)
The most common headache type. Bilateral, pressing or squeezing quality, mild-moderate intensity, associated with pericranial (head and neck) muscle tenderness. Strong evidence for physiotherapy: manual therapy + exercise reduces frequency and intensity. Dry needling of suboccipital and upper trapezius trigger points is particularly effective.
Cervicogenic Headache
Pain originating from the upper cervical spine, referred to the head. Unilateral, provoked by neck movements or sustained posture, radiates from neck to head. Physiotherapy (particularly upper cervical manipulation/mobilisation) is more effective than any other treatment, including medication.
Migraine
Moderate-severe, unilateral, pulsating, associated with nausea/vomiting and light/sound sensitivity. Primarily a neurological condition requiring medical management. However, many migraine patients have co-existing cervical dysfunction that lowers their migraine threshold -- physiotherapy reduces attack frequency by addressing this cervical component.
Cluster Headache
Severe, unilateral, periorbital, with autonomic features (red eye, nasal congestion). Does not respond to physiotherapy -- requires specialist neurological management.
