Gout in India: An Increasingly Common Condition
Gout is a form of inflammatory arthritis caused by deposition of monosodium urate crystals in joints when uric acid levels are chronically elevated. It is the most common inflammatory arthritis in men, and its prevalence is increasing in India due to dietary changes (higher purine intake), alcohol consumption, and metabolic syndrome. The big toe (podagra), ankle, knee, and wrist are most commonly affected.
Acute vs. Chronic Gout Management
Acute gout attacks are managed medically (NSAIDs, colchicine, corticosteroids) and physiotherapy is contraindicated during an acute attack -- the joint is too inflamed and sensitive for manipulation or exercise. However, physiotherapy plays an important role in managing the consequences of recurrent gout: chronic joint inflammation (tophaceous gout), joint damage, and the functional limitations that develop over time.
Physiotherapy for Chronic Gout
Range of Motion Restoration
Recurrent gout attacks lead to progressive joint stiffness. Gentle progressive mobilisation between attacks restores and maintains range of motion in affected joints.
Foot and Ankle Rehabilitation
Big toe (1st MTP) and ankle gout cause altered gait -- painful antalgic gait during and after attacks. Gait retraining and foot strengthening restore normal walking pattern.
Lifestyle Advice
Weight management (obesity increases uric acid production), appropriate exercise (reduces uric acid levels), footwear advice to reduce pressure on affected joints.
