Multiple Sclerosis and Physical Function
Multiple sclerosis (MS) is a chronic autoimmune disease affecting the central nervous system, causing demyelination of nerve fibres and resulting in variable symptoms: fatigue, weakness, spasticity, balance problems, sensory disturbance, and cognitive changes. MS is unpredictable — some patients have a relapsing-remitting course with periods of good function, while others experience progressive decline. Physiotherapy is an essential component of MS management at every stage.
The Role of Exercise in MS
For many years, rest was advised for MS patients due to concerns about overheating and fatigue exacerbation. This advice has been comprehensively overturned by evidence: exercise is safe for MS, does not worsen the disease, and produces significant benefits including: reduced fatigue, improved walking ability, better balance and fall prevention, improved mood and quality of life, and possible neuroprotective effects.
Physiotherapy Interventions for MS
Fatigue Management
Fatigue affects 80% of MS patients and is the most disabling symptom for many. Energy conservation techniques, pacing strategies, and carefully dosed exercise (avoiding overheating) help manage fatigue while maintaining function.
Spasticity Management
Stretching programmes, positioning strategies, and hydrotherapy reduce spasticity. We coordinate with the neurologist for botulinum toxin injections where appropriate.
Balance and Gait Rehabilitation
Progressive balance training, gait retraining, and assistive device assessment reduce fall risk. Walking aids, ankle-foot orthoses, and functional electrical stimulation (FES) for foot drop are all evaluated.
Aquatic Therapy
Warm-pool exercise is particularly appropriate for MS — buoyancy facilitates movement, and careful temperature monitoring prevents heat-related symptom exacerbation.
