Temporomandibular Joint Disorder
The temporomandibular joint (TMJ) is the hinge joint connecting the jaw to the skull. TMJ disorders (TMD) cause pain in the jaw, ear, face, and head; clicking or popping sounds during jaw movement; restricted mouth opening; headaches; and neck pain. TMD is common — affecting approximately 10–15% of adults — and is more prevalent in women aged 20–40.
The TMJ-Neck Connection
An important and frequently overlooked relationship exists between the TMJ and the cervical spine. They share common nerve pathways (trigeminal nerve convergence), and dysfunction in one often affects the other. Many TMD patients have concurrent upper cervical joint dysfunction, and treating the neck can significantly reduce jaw symptoms. We assess both the TMJ and the cervical spine in all TMD patients.
Physiotherapy Treatment for TMD
TMJ Manual Therapy
Gentle mobilisation of the TMJ — including intra-oral techniques (with gloves) — restores normal joint mobility, reduces pain, and improves mouth opening. Disc repositioning techniques are used where appropriate.
Jaw Muscle Therapy
The masseter, temporalis, and pterygoid muscles frequently develop trigger points in TMD patients. Dry needling and soft tissue release of these muscles provides significant pain relief.
Upper Cervical Treatment
Treating upper cervical joint dysfunction and suboccipital muscles concurrently with TMJ treatment produces better outcomes than treating either in isolation.
Posture and Jaw Position Education
Forward head posture changes the resting position of the jaw and increases TMJ stress. Postural correction and advice on jaw resting position (teeth slightly apart, tongue resting on the palate) reduces ongoing joint strain.
