Postural Kyphosis: The Modern Epidemic
Postural kyphosis -- excessive rounding of the thoracic (upper) spine -- has become extremely prevalent in the modern population due to the combination of sedentary work (desk jobs), prolonged phone use, and reduced physical activity. Unlike structural kyphosis (caused by vertebral wedging), postural kyphosis is caused by muscle imbalance and habitual poor posture -- and is significantly correctable with physiotherapy.
Effects of Kyphosis
Thoracic kyphosis has downstream effects throughout the body: neck pain and headaches (forward head posture compensates for rounded upper back), shoulder impingement (altered scapular position impairs rotator cuff function), lower back pain (compensatory lumbar hyperlordosis), reduced lung capacity (compressed thoracic cage), and poor physical appearance affecting confidence.
Physiotherapy Treatment for Postural Kyphosis
Thoracic Mobility
Restoring thoracic extension mobility is the first priority. Foam roller thoracic extension, cat-cow exercises, thoracic rotation, and manual therapy to thoracic facet joints all improve extension range.
Postural Strengthening
Strengthening the muscles that counteract the forward-rounding forces: lower and middle trapezius, rhomboids, serratus anterior, and deep cervical flexors. These muscles actively maintain upright posture throughout the day.
Pectoral and Anterior Chain Stretching
Stretching the tight muscles that contribute to rounded posture: pectoralis minor and major, anterior shoulder capsule, and upper cervical extensors.
Postural Habit Retraining
Movement awareness, postural reminders, and ergonomic modification to break the habitual patterns that perpetuate kyphosis.
