Shoulder surgery -- whether rotator cuff repair, labral repair (Bankart), or shoulder replacement -- is only the first step. The surgical outcome is critically dependent on post-operative physiotherapy.
Rotator Cuff Repair Protocol
The repaired tendon needs 12 weeks to regain adequate tensile strength -- during this period, passive range of motion only. Active movement begins at 12 weeks; strengthening at 16+ weeks. Full recovery: 6-12 months.
Phase 1 (0-6 weeks)
Sling immobilisation, pendulum exercises, passive forward flexion and external rotation.
Phase 2 (6-12 weeks)
Active-assisted range of motion, progressive to full active range. No resisted movements.
Phase 3 (12+ weeks)
Progressive rotator cuff and scapular strengthening, then sport-specific rehabilitation.
