Ankle fractures are serious injuries that typically require 6-12 weeks of non-weight-bearing or partial weight-bearing before bone healing is sufficient for loading. This prolonged immobilisation causes significant ankle stiffness, calf muscle weakness, and proprioception loss.
Rehabilitation Phases
Phase 1: Non-Weight-Bearing (Weeks 0-6)
Ankle circles, foot alphabet exercises, gentle range of motion. Quad and hip exercises to maintain fitness.
Phase 2: Progressive Weight-Bearing (Weeks 6-10)
Gradual transition from partial to full weight-bearing. Ankle range of motion restoration, particularly dorsiflexion. Calf strengthening begins. Walking normalisation.
Phase 3: Strengthening and Proprioception (Weeks 10-16)
Progressive calf strengthening -- single-leg heel raises. Balance and proprioception training on wobble board. Progressive return to normal walking distances.
Phase 4: Return to Sport (Months 4-6+)
Running, jumping, and sport-specific agility drills. Criteria-based return requiring adequate strength, balance, and pain-free performance.
